LIVE FIRING RISK MANAGEMENT FORM
As part of a comprehensive risk management program, we must caution
you that a live-fire session will be used to teach safe gun handling habits
and marksmanship. Of course, every precaution will be taken to prevent
injury, but you have a right to know about the risks involved.
Injury to the eyes can occur which is why all shooters will be provided
with and must wear protective glasses. Firearms using gunpowder have
the potential to damage hearing from exposure to repetitive noise. All
shooters using powder operated guns will be provided with and must
wear hearing protection. Careless shooters have the potential to cause
additional serious injuries, so each shooter will be closely supervised.
Even though shooting is a very safe sport, it is important that you
understand the risks and appreciate how careful we all must be to keep
this way.
I________________________________ understand the risks involved and
the need to listen for and follow directions. DATE:___________________
I understand the risks involved and grant permission for my child to
participate in the live-fire sessions. In the event of illness or injury, I
grant permission for my child to receive medical treatment.
